Kolesteroolitaseme alandamiseks tasub eelistada kvaliteetseid rasvu, s\u00fc\u00fca lahustuvaid kiudaineid, liikuda regulaarselt, hoida kehakaal kontrolli all, loobuda suitsetamisest ja piirata alkoholi tarvitamist. Eesti juhistes soovitatakse hoida \u00fcldkolesterool alla 5 mmol\/l ja LDL-kolesterool alla 3 mmol\/l; s\u00fcdameinfarkti p\u00f5denud v\u00f5i diabeediga inimestel on sihtv\u00e4\u00e4rtused veelgi madalamad (1).<\/p><p>Kolesteroolitasemele hakatakse sageli m\u00f5tlema alles siis, kui vereanal\u00fc\u00fcs n\u00e4itab, et see on k\u00f5rge. Enamasti ei p\u00f5hjusta k\u00f5rge kolesterool enesetundes m\u00e4rgatavaid s\u00fcmptomeid, kuid liigne LDL-kolesterool v\u00f5ib tasapisi ladestuda veresoonte seintele. Arsti kabinetis soovitatakse tavaliselt esmalt teha muutusi elustiilis ning vajaduse korral lisada hiljem ravimeid. See artikkel selgitab, mida k\u00f5rge kolesterool t\u00e4hendab, miks LDL-il ja HDL-il on erinev roll, kuidas kolesteroolitaset m\u00f5jutada toitumise ja elustiilimuutustega ning millal tuleks pidada n\u00f5u tervishoiut\u00f6\u00f6tajaga.<\/p><h2>L\u00fchivastus<\/h2><p>Kolesterooli alandamine keskendub peamiselt LDL-kolesterooli v\u00e4hendamisele, sest k\u00f5rge LDL soodustab naastude teket arterite seintele ning suurendab s\u00fcdameinfarkti ja insuldi riski (2). Peamised sammud on k\u00fcllastunud ja transrasvade v\u00e4hendamine, lahustuvate kiudainete tarbimise suurendamine, k\u00fcllastumata rasvade eelistamine, regulaarne liikumine, kehakaalu hoidmine kontrolli all, suitsetamisest loobumine ja alkoholi m\u00f5\u00f5dukas tarvitamine (3). Kui LDL-kolesterool p\u00fcsib k\u00f5rge v\u00f5i riskitegureid on mitu, tasub raviplaan koostada koos arstiga, sest m\u00f5nel juhul ei pruugi elustiilimuutustest \u00fcksi piisata.<\/p><h2>Mis on kolesterool ja miks see on vajalik?<\/h2><p>Kolesterool on vahajas, rasvasarnane aine, mida organism vajab rakumembraanide ehitamiseks, teatud hormoonide s\u00fcnteesiks, sapphapete tootmiseks ja D-vitamiini ainevahetuseks. Maks toodab suure osa kolesteroolist ise, kuid osa saadakse ka toidust. Kolesterool liigub veres lipoproteiinide abil ning selle taset m\u00f5\u00f5detakse vereanal\u00fc\u00fcsiga. Kuigi organism vajab kolesterooli, tekib probleem siis, kui LDL-kolesterooli tase veres on liiga k\u00f5rge v\u00f5i kui eri kolesteroolivormide tasakaal muutub ebasoodsaks. Nii k\u00f5rgenenud LDL-kolesterool kui ka liiga madal HDL-kolesterool v\u00f5ivad suurendada s\u00fcdame- ja veresoonkonnahaiguste riski (2).<\/p><h2>LDL ja HDL: hea ja halb kolesterool<\/h2><p>Kolesterool ei liigu veres vabalt, vaid on seotud lipoproteiinidega. K\u00f5ige tuntumad neist on kaks fraktsiooni: madala tihedusega lipoproteiin ehk LDL ja suure tihedusega lipoproteiin ehk HDL.<\/p><p>LDL-kolesterool transpordib kolesterooli kudedesse. Kui LDL-i on veres liiga palju, v\u00f5ib see ladestuda arterite seintele ning moodustada rasvast ja kaltsiumist koosnevaid naastusid, mis ahendavad veresooni. See protsess suurendab s\u00fcdameinfarkti ja insuldi riski (2). Seet\u00f5ttu nimetatakse LDL-i sageli \u201ehalvaks kolesterooliks\u201c.<\/p><p>HDL-kolesterool viib liigse kolesterooli kudedest ja veresoontest tagasi maksa, kus see muudetakse sapphapeteks v\u00f5i eemaldatakse organismist. Piisav HDL-i tase aitab v\u00e4hendada s\u00fcdameriski, mist\u00f5ttu nimetatakse HDL-i \u201eheaks kolesterooliks\u201c (2). Siiski r\u00f5hutavad t\u00e4nap\u00e4evased ravijuhised, et t\u00e4helepanu peaks olema eelk\u00f5ige LDL-i langetamisel, sest LDL-i ja s\u00fcdame-veresoonkonnahaiguste vaheline seos on k\u00f5ige selgem.<\/p><h2>Mis on kolesterooli norm?<\/h2><p>Kolesteroolitasemete hindamisel kasutatakse Eestis \u00fchikuna mmol\/l. Ida-Tallinna Keskhaigla infomaterjalis on tervetele t\u00e4iskasvanutele esitatud j\u00e4rgmised soovituslikud v\u00e4\u00e4rtused: \u00fcldkolesterool alla 5,0 mmol\/l, LDL-kolesterool alla 3,0 mmol\/l, HDL-kolesterool \u00fcle 1,0 mmol\/l ja trigl\u00fctseriidide tase alla 1,7 mmol\/l (1). Sobiv sihttase s\u00f5ltub vanusest, soost, perekondlikust riskist, verer\u00f5hust, veresuhkru tasemest ja kaasuvatest haigustest. Arst v\u00f5ib m\u00e4\u00e4rata rangemad eesm\u00e4rgid, kui risk on k\u00f5rge v\u00f5i kui inimesel on esinenud s\u00fcdame v\u00f5i ajuinfarkt.<\/p><h2>Kuidas testida enda s\u00fcdame veresoonkonna riski?<\/h2><p>Lisaks vereanal\u00fc\u00fcsile soovitab Ida-Tallinna Keskhaigla hinnata s\u00fcdame-veresoonkonnahaiguste riski ka v\u00f6\u00f6\u00fcmberm\u00f5\u00f5du m\u00f5\u00f5tmisega. Rasv koguneb kehas eri piirkondadesse: kui see ladestub peamiselt puusadele ja istmikule, sarnaneb keha \u201epirniga\u201c, kuid k\u00f5hu\u00f5\u00f5nde kogunev rasv tekitab \u201e\u00f5unakujulise\u201c kehavormi. \u00d5unakujulise kehaga inimestel on suurem risk haigestuda s\u00fcdame-veresoonkonnahaigustesse ja diabeeti, sest k\u00f5hu\u00f5\u00f5nes paiknevad rasvarakud toodavad bioaktiivseid aineid, mis soodustavad ateroskleroosi ja p\u00f5letikku (1).<\/p><p>V\u00f6\u00f6koha m\u00f5\u00f5tmiseks v\u00f5tke s\u00e4rk seljast ja l\u00f5dvestage v\u00f6\u00f6rihm. Asetage m\u00f5\u00f5dulint \u00fcmber keskkoha nii, et see paikneks v\u00f5rdsel kaugusel puusaluust ja viimase roide alaservast. M\u00f5\u00f5tke p\u00e4rast v\u00e4ljahingamist. Risk suureneb, kui talje\u00fcmberm\u00f5\u00f5t on meestel v\u00e4hemalt 94 cm ja naistel v\u00e4hemalt 80 cm (1). Kui m\u00f5\u00f5tmistulemus \u00fcletab neid piire, tasub p\u00f6\u00f6rduda perearsti poole ja arutada, millised elustiilimuutused v\u00f5i uuringud v\u00f5iksid riski v\u00e4hendada.<\/p><h2>Miks k\u00f5rge kolesterool on ohtlik?<\/h2><p>Kui veres on liiga palju LDL-kolesterooli, hakkab see ladestuma arterite siseseintele. Aja jooksul moodustuvad kolesteroolist, kaltsiumist ja sidekoest koosnevad naastud, mis muudavad veresooned j\u00e4igemaks ja kitsamaks. Seda protsessi nimetatakse ateroskleroosiks. Kitsenenud arterid v\u00e4hendavad verevoolu s\u00fcdamesse, ajju ja teistesse elunditesse. Kui naast rebeneb ja tekib tromb, v\u00f5ib verevool t\u00e4ielikult sulguda ning tagaj\u00e4rjeks v\u00f5ib olla s\u00fcdameinfarkt v\u00f5i insult. K\u00f5rge kolesterool on ohtlik eesk\u00e4tt seet\u00f5ttu, et suurendab pikema aja jooksul s\u00fcdame- ja veresoonkonnat\u00fcsistuste riski. Parim tulemus saavutatakse siis, kui kombineeritakse mitmeid s\u00fcdames\u00f5bralikke harjumusi. J\u00e4rgnevad alapeat\u00fckid selgitavad, kuidas toitumine, liikumine, kehakaal ja muud tegurid m\u00f5jutavad LDL kolesterooli ning annavad praktilisi n\u00e4pun\u00e4iteid.<\/p><h3>1. V\u00e4henda k\u00fcllastunud ja transrasvu<\/h3><p>Suurim muutus, mida toiduvalikus teha saab, on v\u00e4hendada k\u00fcllastunud ja transrasvade osakaalu. Uuringud n\u00e4itavad, et k\u00fcllastunud rasvhapped (nt v\u00f5i, rasvane liha, t\u00e4ispiim, palmi- ja kookos\u00f5li) t\u00f5stavad LDL-kolesterooli, sest v\u00e4hendavad maksa LDL-retseptorite aktiivsust ja suurendavad LDL-i tootmist (4). Transrasvad, mis on osaliselt h\u00fcdrogeenitud taimerasvad ja mida leidus varem margariinides, k\u00fcpsetistes ning friteeritud toitudes, t\u00f5stavad LDL-i ja langetavad HDL-i (4).<\/p><p>Toitumisspetsialistid r\u00f5hutavad, et k\u00fcllastunud rasvade tarbimist tuleks piirata ja asendada need tervislikumate rasvadega (5). Punase liha asemel v\u00f5ib sagedamini valida kala, nahata linnuliha v\u00f5i kaunvilju ning v\u00f5i ja vahukoore asemel kasutada oliivi- v\u00f5i rapsi\u00f5li. Transrasvu tasub t\u00e4ielikult v\u00e4ltida. Kuigi osaliselt h\u00fcdrogeenitud \u00f5lid on mitmel pool keelustatud, v\u00f5ib m\u00f5nes pakendatud toidus siiski olla v\u00e4ike kogus transrasvu, seega tasub toiduainete etikette hoolikalt lugeda (5).<\/p><h3>2. Suurenda lahustuvate kiudainete tarbimist<\/h3><p>Lahustuvad kiudained seovad seedetraktis sapphappeid ja v\u00e4hendavad kolesterooli imendumist. Mitmed teadus\u00fclevaated n\u00e4itavad, et nii kiudained kui ka taimsetes steroolides leiduvad \u00fchendid aitavad LDL-kolesterooli v\u00e4hendada, aeglustades kolesterooli imendumist sooles (4). Head allikad on kaerahelbed, oad, l\u00e4\u00e4tsed, herned, \u00f5unad ja pirnid. Hommikune kaerapuder v\u00f5i l\u00f5unasupile lisatud l\u00e4\u00e4tsed aitavad suurendada kiudainete hulka men\u00fc\u00fcs ning hoiavad samal ajal k\u00f5hu kauem t\u00e4is.<\/p><p>Lahustuvaid kiudaineid soovitatakse tarbida v\u00e4hemalt 10\u201325 grammi p\u00e4evas. <a href=\"https:\/\/lotos-pharma.ee\/millised-toidud-alandavad-kolesterooli\/\">T\u00e4psema toitude nimekirja leiab artiklist \u201eKolesterooli alandavad toidud\u201c.<\/a><\/p><h3>3. Eelista k\u00fcllastumata rasvu<\/h3><p>K\u00f5ik rasvad ei ole kahjulikud. Oluline on nende kvaliteet: kui k\u00fcllastunud rasvhapped asendada monok\u00fcllastumata rasvhapetega (nt oliivi- ja rapsi\u00f5li, avokaado, p\u00e4hklid, seemned) v\u00f5i pol\u00fck\u00fcllastumata rasvhapetega (nt p\u00e4evalille- v\u00f5i maisi\u00f5li, rasvane kala), v\u00f5ib LDL-i tase alaneda. Teadusuuringute kohaselt suurendavad monok\u00fcllastumata ja pol\u00fck\u00fcllastumata rasvhapped maksa LDL-retseptorite aktiivsust ning alandavad seel\u00e4bi LDL-kolesterooli (4). Praktikas t\u00e4hendab see, et salatikastme v\u00f5iks valmistada oliivi\u00f5list, p\u00e4hkleid v\u00f5iks s\u00fc\u00fca vahepalaks ning rasvane kala, n\u00e4iteks l\u00f5he, heeringas v\u00f5i makrell, v\u00f5iks j\u00f5uda toidulauale paar korda n\u00e4dalas.<\/p><p>Transrasvad ja rafineeritud s\u00fcsivesikud ei ole head asendajad, sest need v\u00f5ivad hoopis t\u00f5sta trigl\u00fctseriidide ja LDL-i taset veres. Seet\u00f5ttu tasub m\u00f5elda, kuidas asendada k\u00fcpsetistes v\u00f5i ja margariin taimsete \u00f5lidega ning v\u00e4hendada kommide ja saiakeste osakaalu.<\/p><h3>4. Liigu regulaarselt<\/h3><p>Kehaline aktiivsus aitab t\u00f5sta HDL-kolesterooli, v\u00e4hendada trigl\u00fctseriidide taset ja hoida kehakaalu kontrolli all. Soovitatav on liikuda v\u00e4hemalt 30 minutit m\u00f5\u00f5duka intensiivsusega viis korda n\u00e4dalas v\u00f5i teha v\u00e4hemalt 25 minutit intensiivset treeningut kolm korda n\u00e4dalas (5). Sobivad n\u00e4iteks kiire k\u00f5nd, rattas\u00f5it, ujumine, tantsimine v\u00f5i j\u00f5utreening \u2013 oluline on leida tegevus, mis meeldib.<\/p><p>F\u00fc\u00fcsilise aktiivsuse m\u00f5ju lipiididele on k\u00e4sitletud mitmes koondanal\u00fc\u00fcsis. Uuringute p\u00f5hjal seostub kehaline aktiivsus HDL-kolesterooli keskmiselt umbes 4,6% t\u00f5usuga, LDL-kolesterooli umbes 3,7% langusega ja trigl\u00fctseriidide umbes 5% v\u00e4henemisega (6). \u00dcldkolesterooli tase ei pruugi oluliselt muutuda, kuid HDL-i ja LDL-i suhe paraneb. Isegi l\u00fchikesed liikumispausid p\u00e4eva jooksul \u2013 trepist k\u00f5ndimine, jalutusk\u00e4ik t\u00f6\u00f6pausi ajal v\u00f5i jalgrattaga poodi s\u00f5itmine \u2013 aitavad n\u00e4dalase liikumisaja t\u00e4is saada.<\/p><h3>5. Hoia kehakaal kontrolli all<\/h3><p>Liigne kehakaal, eriti k\u00f5hupiirkonnas, m\u00f5jutab seda, kuidas organism rasvu ja kolesterooli t\u00f6\u00f6tleb. Isegi m\u00f5\u00f5dukas kaalulangus v\u00f5ib parandada LDL-i, HDL-i ja trigl\u00fctseriidide tasakaalu. Kaalulangust saab alustada v\u00e4ikeste sammudega, n\u00e4iteks asendades magusad joogid veega, valides tervislikumaid suupisteid ja j\u00e4lgides portsjonite suurust (5). Liikumise suurendamine aitab kulutada energiat ja s\u00e4ilitada lihasmassi, mis omakorda toetab ainevahetust.<\/p><p>Toitumine ja kaalulangus ei t\u00e4henda ranget n\u00e4ljutamist. Paljudele sobib paremini toitumismuster, mis r\u00f5hutab k\u00f6\u00f6givilju, t\u00e4isteratooteid, puuvilju, lahjasid valguallikaid ja tervislikke rasvu. Oluline on leida p\u00fcsiv l\u00e4henemine, mida suudad j\u00e4rgida kuude ja aastate jooksul.<\/p><h3>6. Loobu suitsetamisest<\/h3><p>Suitsetamine kahjustab veresoonte sisekihti ja m\u00f5jutab otseselt lipiidiprofiili. Suitsetamine alandab HDL-kolesterooli, eriti naistel, ning t\u00f5stab LDL-kolesterooli (3). Sigaretisuits sisaldab kemikaale, mis oks\u00fcdeerivad LDL-i, muutes selle kleepuvamaks ja soodustades naastude teket. Suitsetamisest loobumine parandab vereringet ja t\u00f5stab HDL-i taset juba m\u00f5ne kuu jooksul. See ei ole pelgalt tahtej\u00f5u k\u00fcsimus \u2013 vajaduse korral tasub kasutada nikotiinasendusravi, n\u00f5ustamist v\u00f5i toetusr\u00fchmi.<\/p><h3>7. Piira alkoholi<\/h3><p>Alkoholi m\u00f5ju kolesteroolile on vastuoluline. M\u00f5ned uuringud viitavad sellele, et m\u00f5\u00f5dukas alkoholitarbimine v\u00f5ib t\u00f5sta HDL-kolesterooli taset, kuid sellest ei piisa, et soovitada inimestel alkoholi tarvitama hakata. Liigne alkoholitarbimine, st naistel rohkem kui \u00fcks ja meestel rohkem kui kaks alkohoolset jooki p\u00e4evas, v\u00f5ib t\u00f5sta \u00fcldkolesterooli taset, suurendada trigl\u00fctseriidide hulka ja t\u00f5sta verer\u00f5hku (3, 5).
